M44 – Rapid Response Needs Survey/Wait List

If you are a Stage IV cancer patient in need of help, you can join MoreFor4’s Wait List by completing this Needs Survey. You will be asked to answer questions about your financial and other service/support needs. This survey does NOT require PROOF of diagnosis or income level/sources, although documentation will be necessary later — once you move into M44’s formal Social Work TRIAGE intake process. (That will not happen until we have financial resources to help you and other cancer patients. NO FUNDS ARE AVAILABLE YET, and joining the Wait List does not mean you have been approved for financial aid. The next stage of this process will determine your eligibility.)

By completing this survey, you will be added to M44’s Wait List and get email notifications when assistance funds and other support programs become available, plus instructions on how to start your Rapid Response intake process once we are ready for you. But, by participating in this survey, you are ALSO helping us create a “Demonstration of Need.” In other words, by filling out this form, you will help us prove to potential funders: 1) How many people need help, 2) The kind of help they need, and 3) How much money it would take to help everyone on the Wait List.

You are not required to join the Wait List in order to receive help, but those who sign-up will receive priority status once patient assistance funds and other support services become available.

The survey begins with a short, 3-question Eligibility Quiz.
If you meet these basic standards, you will automatically advance to the full survey tool.

PS: While we really do want cancer patients to apply directly… and tell us what is important to them and how they view their current challenges and needs, we know that sometimes people need help filling out things like this. Social Workers and other healthcare professionals may assist someone in completing a Needs Survey, as can a friend or family member. Since a family member or healthcare professional may not always answer the same way a Stage IV cancer patient may about themselves (in fact, we find this is often the case), we encourage you to fill this out together and be sure the answers reflect what the individual with Stage IV cancer thinks about all this… rather than what you (as the “Referrer”) interpret as their desires and needs. Also, if you are a Stage IV cancer patient and need assistance in filling out this survey, we will help you. M44’s First 5 “Cancer… AND” Support Groups

MoreFor4’s Support Groups follow our unique “CANCER… AND” model. Rather than focus on the medical side of cancer and treatment, M44-sponsored groups GO BEYOND diagnosis alone and aim to offer targeted support for MoreCorps members with distinctly different needs and concerns. (Our online MoreCorps community platform – set to launch in early 2027 – will allow members to start their own Support Groups as well. However, M44-sponsored groups will follow this model, to ensure people can find groups that “FIT” who they are, what they want, what they value, and what they… NEED.)

The first step is to pre-register for one or more groups. Please note that the form allows you to tell us what you need EVEN IF the groups listed below are not what you seek. We ask that you answer questions about your age group, race, and other things so we can determine how many groups we will need to schedule. For example, young adults with Stage IV cancer may want to be in a group with people around the same age. Same with older folks. And the same with people with different intersectional identities.

By pre-registering, you can help us plan for the launch of each of these initial Support Groups (and help us determine what other groups should be introduced early-on). Additionally, by pre-registering, you will help M44 prove to potential funders that there is, indeed, a need and DEMAND for our Support Groups. Learn more by clicking the link below.
